Important Forms

Each Player is required to submit a sports physical signed by a licensed medical professional each year. Players will be prohibited from participating in seasonal activities until this form is submitted to the Lancaster High School Athletic Department.

Every activity that your player participates in school requires an Emergency Medical Form. The coaches keep these with them at all sporting events. Players will be prohibited from participating in seasonal activities until this form is submitted to the coaching staff.  Print the Emergency Medical form using the link at the bottom of this page.

For the June and July months we are without busing and the transportation of the players falls to the parents. Whether you are planning on transporting your player or participating as a carpool parent, the waiver must be filled out and turned into the coach before an event. All players are required to submit a completed form.   Print the Transportation Waiver using the link at the bottom of this page.
